Everything you need to know before your first visit
Physical therapy is a healthcare profession dedicated to improving and restoring movement and function. Physical therapists are licensed healthcare professionals who diagnose and treat individuals of all ages who have medical conditions, illnesses, or injuries that limit their ability to move and perform functional activities in their daily lives.
Physical therapists examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, physical therapists work with individuals to prevent the loss of mobility before it occurs by developing fitness and wellness programs for healthier and more active lifestyles.
Physical therapy services include evaluation and assessment, manual therapy, therapeutic exercise, neuromuscular re-education, ultrasound, electrical stimulation, hot and cold therapies, and patient education.

To save time at your first appointment, please download and complete the patient intake forms before you arrive. Bring the completed forms with you or ask our staff about submitting them in advance.
The forms include your personal and insurance information, medical history, and a description of your current condition. Completing them in advance allows your therapist to spend more time focused on your evaluation and treatment.
